Scotland Wide Free Bus Pass

Licence revoked due to medical reasons

To apply for a free bus pass if you have had a driving license you need:

  • A free bus pass application form and a certificate of eligibility form (available by calling Epilepsy Scotland on 0808 800 2200 and from your local authority - or the Travel Card Unit if you live in Strathclyde).
  • The certificate of eligibility form can be signed by your GP, Epilepsy Specialist Nurse or hospital consultant.
  • Proof of address acceptable documents include: council tax bill (current financial year), utility bill e.g. gas, electricity, landline telephone (within last 3 months), bank statement (within last 3 months), TV licence, letter from residential care home, letter from home office or immigration office or letter from local authority confirming that you stay in the council area.
  • 1 passport photograph (taken within the last year) Print your name and application reference number on the back of the photograph.
  • Please be aware each year you will need to renew your bus pass in the same way. Do this early as it can take time to process.
