In receipt of Disability Living Allowance (DLA
To apply for free bus travel in Scotland if you receive Disability Living Allowance (high mobility or high or middle rate care) you need:
- A free bus pass application form (available by calling Epilepsy Scotland on 0808 800 2200)
- Disability Living Allowance award letter issued within the last 12 months

Text phone: 08457 22 44 33 - Proof of address acceptable documents include: council tax bill (current financial year), utility bill e.g. gas, electricity, landline telephone (within last 3 months), bank statement (within last 3 months), TV licence, letter from residential care home, letter from home office or immigration office or letter from local authority confirming that you stay in the council area.
- 1 passport photograph (taken within the last year) Print your name and application reference number on the back of the photograph.
- If you are in receipt or have been awarded higher or middle rate of the care component of DLA or Attendance Allowance you may be entitled to a National Entitlement Card containing the companion entitlement (C+1).
